Initial session for 60 minutes: $200
Subsequent sessions for 50 minutes: $130
Cash and Veteran discounted rate for initial 60 minute session: $150
Cash and Veteran discounted rate for 50 minutes subsequent sessions: $100

Insurance
I am currently onboarding with the following insurance companies:
Moda Health
Blue Cross Blue Shield
I am happy to provide super bills for individuals who are out-of-network, to be used for reimbursement purposes. If your insurance company provides out-of-network coverage, please contact them to determine levels of coverage, as they are often lower than for in-network services.
I do not bill insurance at this time, though will gladly bill the above-mentioned insurance companies when the credentialing process has been completed.
Insurance Considerations
Insurance companies only reimburse for services and diagnoses that they deem medically necessary. If you choose to use insurance, which makes sense financially for many individuals, keep in mind that I will have to provide dates of service and diagnosis for coverage. Furthermore, they may ask for additional documentation to substantiate medical necessity.
No Show and Late Cancellation Policy
If you need to cancel, please give a 24 hour notice. If you late cancel or no-show, and it becomes a pattern, it may be necessary for you to sign an attendance agreement, requiring you to pay a fee for missed appointments. This fee is not billable to insurance and the full fee will be your responsibility. Furthermore, repeated missed appointments may result in therapy services no longer being offered.
If you are late for a session, you may lose some of that session time. Sessions are not extended if you are late.
